Dental
Services Registration Form
The Bristol Virginia Health Department, in conjunction with Bristol Virginia Schools, is making Dental Services available for those students who qualify. The student must have Medicaid or FAMIS Insurance or participate in the reduced-fee lunch program to be eligible. Parents will be responsible for transporting their child/children to the dental clinic, which will be located at Virginia Middle School near Girls, Inc. and across the street from the Hillbilly Market. If a parent is not able to be present at the time of services, please contact your school nurse to receive and complete the Dental History Sheet. Please return this sheet to you school nurse. No services can be provided without this sheet. If you have questions or need further information, please contact your school nurse. Parents must call the Dental Clinic to schedule an appointment for their child/children. Appointments are required.
